This taxonomy has been used extensively by the health field, including nursing, to structure teaching plans and outcome testing. Blooms Taxonomy, consists of a hierarchy within 3 different domains of learning: cognitive, affective, and psychomotor (Larkin Burton, 2008). According to the taxonomy there are several subcategories within the cognitive domain. The lowest aspect in the hierarchy for the cognitive domain is knowledge. One is expected to retrieve information from long-term memory. Using this in nursing would be the patient being able to recognize medication side effects associated with their medication regimen. Advancing up the pyramid is comprehension. This is when one is expected to construct meaning from oral, written, or graphic information. Using this in nursing the patient would be able to explain the importance of having clean hands while doing central line flushing. Application is next. Polycystic ovary syndrome (PCOS), also known as Stein Leventhal Syndrome, was first described in 1935. PCOS, which may also be referred to as polycystic ovary disease (PCOD) is the most common hormonal disorder found in premenopausal women. PCOS affects 7% of women from all races and nationalities. Typically, PCOS symptoms first appear in adolescence, normally around the state of menstruation. Occasionally, some women do not develop PCOS symptoms until their early to mid-20s. One of the most common symptoms of PCOS is irregular periods. Polycystic ovary syndrome (PCOS) becomes symptomatic during adolescence and affects at least 5% of reproductive-age women. PCOS is a heterogeneous syndrome of unexplained chronic hyperandrogenism and oligo-anovulation, with a polycystic ovary being an alternative diagnostic criterion. About half of cases lack some of the classic Stein-Leventhal syndrome features of menstrual irregularity, hirsutism, obesity, and polycystic ovaries. Whether the syndrome can be diagnosed in the absence of hyperandrogenism is controversial, and the documentation of hyperandrogenemia can be problematic. The broad spectrum of the disorder seems to encompass atypical cases of hyperandrogenemia with central obesity and features of insulin resistance instead of hirsutis m or anovulation. Functional ovarian hyperandrogenism (FOH) is usually the source of the androgen excess. It is characterized by 17-hydroxyprogesterone (17PROG) hyperresponsiveness to the gonadotropin stimulation of GnRH agonist or human chorionic gonadotropin testing and subnormal suppressibility of plasma testosterone upon adrenal suppression by glucocorticoid. It is often accompanied by functional adrenal hyperandrogenism, characterized by 17-hydroxypregnenolone or dehydroepiandrosterone (DHEA) hyperresponsiveness to ACTH; in atypical PCOS, the sole source of androgen excess may be functional adrenal hyperandrogenism. The underlying defect seems to be a constitutive dysreglulaton of steroidogenic cells (9). Intrinsic granulosa cell dysfunction is also present. LH excess appears to contribute to 50-75% of cases. Evidence is accumulating that it arises from androgen interfering with the progesterone negative-feedback effect on LH secretion. However, the possibility of primary central mechanisms for L H excess remains, particularly in adolescent PCOS . Increasing evidence suggests that PCOS arises as a complex trait with contributions from both heritable and nonheritable factors. Polygenic influences appear to account for about 70% of the variance in pathogenesis. Nearly half of sisters of women with PCOS have an elevated plasma testosterone level, although only half of them are symptomatic. Polycystic ovaries appear to be transmitted as a dominant trait, usually asymptomatic but often accompanied by a subclinical PCOS type of ovarian dysfunction. Central obesity and insulin resistance seem to play important roles in PCOS, perhaps by accentuating steroidogenic dysregulation but perhaps more fundamentally because PCOS is closely related to these features in parents. Gestational factors have also been incriminated; the syndrome has been associated with high birth weight in heavy mothers and can arise from fetal programming by androgen excess. In view of these indications for congenital origins of the syndrome, it is not surprising th at there is increasing recognition of risk factors for PCOS in childhood Minerva Pediatr (2010) Polycystic Ovary syndrome (PCOS) is a complex disorder, involving primarily ovarian hyperandrogenism in females and linked with insulin resistance in the majority of cases. Clinical features are widely variable and include a combination of menstrual irregularities, acne, hirsutism, and alopecia. Although it typically presents around puberty, several risk factors during childhood may help raise a high index of suspicion for the development of PCOS in adolescents. The pathophysiology of PCOS still remains unknown and likely includes a combination of genetic factors, insulin resistance and environmental factors. A thorough diagnostic work up is required in suspected cases and several management modalities have been suggested. Since various long term complications and comorbidities are associated with PCOS early diagnosis and therapeutic intervention is warranted in these cases. The aim of this study was to determine prevalence of polycystic ovaries (PCO) and associated clinical and biochemical features among women with infertility attending gynaecological outpatients department (GOPD) at Muhimbili National Hospital (MNH) in Dar es Salaam, Tanzania. All women with infertility attending the GOPD from 11th September 2006 to 15th February 2007 were recruited to the study. Information on socio-demographic, obstetric and menstrual characteristics was collected. Anthropometric measurement, clinical examination of acne and hirsutism, vaginal ultrasonography for PCO and biochemical analysis of luteinizing hormone (LH), follicle stimulating hormone (FSH) and testosterone were performed all 102 women who attended the GOPD during the study period due to infertilityw ere recruited. Two women were excluded after diagnosis of pregnancy made by hormonal assay and ultrasonography thus remaining with 100 women for analysis. Oligomenorrhoea and acne were significantly higher in a group of women with PCO than among women with normal ovaries. The mean hirsutism score though was not significant, was higher in women with PCO than in women with normal ovaries (5.1 +/-2.7 vs 4+/-2.4, P Koivunen R. (1999) conducted study about prevalence of polycystic ovaries in healthy women. The prevalence of polycystic ovaries varies with age. The findings are more common in women aged 35 years or younger than in those aged 36 years or older. Women with PCOS were more likely to have fears about future health related to weight gain (p=0.045 ), loss of femininity (p =0.035) , loss of sexuality (p =0.003) and infertility (p=0.019) than women without PCOS. Worsened quality of life, anxiety and depression in young women with PCOS is related to BMI. The success from this education, we can see from the quality of the education, we know that United States is very popular in education, for example so many people from United States can work in out of this country easily after finishing their school. The system of education here is very good, in which studying in this country is very nice because we can study what we want without taking so many general educations. For example when we want to study business Computer information system, we can finish it by 3 years. But other country like Indonesia, we cannot graduate just by 3 years only because so many general educations we have to take there. So it is not efficient to take many General educations because it is not important, anyway we will forget it after we finish school. On the other hand American education has some disadvantages, for example like Asian students can do better than American students, it is really crazy things, because Asian students have to study the language first before they study here. But in fact Asian students are better. We can see why Asian students are better because of 2 sides, in which it is intern side and extern side. Intern side is for example like the motivation of their parents, and Confucianism. And extern side is the quality of their high school in Asian. So that is why Asian Students do not feel difficulties in studying here. For example like in Singapore in which the standard of the high school there is the highest in this world for math, so the students from Singapore who want to continue their study here they will not feel any difficulties because the level of difficulties in almost all subject is lower than in Singapore. In my experience, I can decide that American education is very good, because we just take like 3 years to take bachelor, and after that we can work to look for money, and it is really good thing, but American system has failure also, for example in my math class, all Asian students can get A, we can see it how bad the quality of high school here, because almost all students have to study new things in university, but in fact all Asian students have already studied that thing in their high school in Asia. Blanche as a character although resembling, at times, the potential to be of more substantial character and command the recognition she deserves, is trapped into a bubble of what can be considered feminine and is convinced by her own sub conscience and those around her that the only way to get what she wants is to act within certain stereotypes to find herself any man to provide the stability she craves, Stella alike takes the role in a less exaggerated manner but this in some ways is more powerful as she has the power over Stanley to conduct him as she wishes but herself deems it only acceptable to run back to him every time he calls. Self-destruction in its simplest form because society has condemned her to our four walls of femininity. A Streetcar Named Desire presents a sharp observational critique of the way the institutions and attitudes of postwar America placed restrictions on women’s actions and lives. Williams uses Blanche’s and Stella’s dependence on men to expose and evaluate the treatment of women during the transition from the old to the new South. Both Blanche and Stella see male companions as their only means to achieve happiness, Blanche and Stella throughout the play remain in the mindset that to acquire a male companion is their only true and justified path to happiness, consequently they form a dependency to men for both their sustenance and their self-image. Blanche criticizes Stella for staying in a physically abusive relationship with her brooding husband Stanley, thus in turn criticizing Stella for depending on Stanley for emotional support and sexual relief, Stella declares she is ‘not in anything she wants to get out of’ this moment in the play shocks Blanche as she realises that someone she loves and respects so completely could choose this life for herself . However the way Blanche leads her life—contacting Shep Huntleigh for financial support—still demonstrates a complete uncompromising dependency on men. At the end of the play, when Stella makes the conscious decision to remain with her husband, Stanley, she has chosen to rely on love, and put her faith and reliance in a man instead of her sister. Williams chooses not to present this decision as a mistake or a female falling as it is clear that Blanche’s behavior does not offer nor demonstrate a secure future for Stella or for her newly born child. One of the most prominent communications within the play is that posed by Blanche, as a character we find it incredibly hard to sympathize with her, she appears on the surface manipulative and unhinged masked clumsily by nauseating girlishness . She comes across in so many ways barely a woman at all with her shrill demands and constant awkward giggling, however her past reveals she is far more of a woman than Stella or Blanche would ever like to believe, her shaded life in Bellereeve leaves trails of fragmented womanhood behind her which later manifests itself in the form of vicious gossip that consequently sparks Blanches downfall. Within herself, Blanche views a quick marriage to Mitch as a manner of absconding destitution. Blanche’s sexual exuberance is criticized by Stanley and exploited by all the other men in her life excluding Mitch, tarnishing her name, and providing her with a promiscuous reputation. This reputation, by nature of society, brands Blanche an unfit and unattractive marriage prospect, however, as she is destitute, Blanche perceives marriage as her only probability for survival in a bible belt that condemns women be pure in order to be a possibility for wedlock. When Mitch discards Blanche based on the slander painted by Stanley about her licentious past, Blanche instantaneously turns to another man—the millionaire Shep Huntleigh—in hope of another miraculous rescuing. Because Blanche is blinded by her dependence on men, she loses sight of a realistic concept of saving herself from being pushed down, this view has been embedded in her by humanity condemning her to believe her only discharge will be if a good honest man will wed her, thus somehow erasing the shadows of her past in Bellereeve. Blanche fails to come to terms with the fact that by putting so much emphasis and reliance in men she no longer controls the outcomes of her future as she puts her fate in the hands of a man, thus ultimately leading to her descent into insanity. One of the most important aspects of feminism is that gender is a social construct and if womanhood is defined purely through the chains of society rather than through natural causes, few societies have enforced it more forcefully and with such vigour as the American South. The saga that is the southern woman began in the mid 1800’s which saw a white woman of a certain standing put on a pedestal. The southern gentleman and society enforced that a woman be a non-sexual creature, helpless and fragile, this is concentrated and highlighted in Blanche’s behaviour as she believes the only way for her to appear attractive to the opposite sex is to manifest herself as pathetic and incapable. As a Southern lady, Blanche’s narrowly defined social role has kept her from admitting her natural appetites and pursuing them forthrightly. She has felt obliged to lie to herself and to others. However throughout the play these hidden desires have revealed themselves in private company, for instance her heavy drinking would be seen as impure by southern society and this is evident by both Mitch’s and Stanley’s reaction to this trait. Blanches greatest scandal from belle reeve is that she let her sexual nature control her actions thus ‘spoiling’ her and rendering her a ruined woman. To express one’s sexuality or desires instantaneously reduces a woman to tainted and impure. Williams has said that he considers Blanche’s character liberated, she has lived such an autonomous life in such a repressive time she could almost be considered heroic were she not so ashamed of her fierce former independence. She has not proved a weak adversary at any moment and at the climax of the play smashes a bottle and threatens to ‘’twist it into his face’’. Stanley does not rape Blanche in order to re-instate his power in the house rather than as a battle for position of alpha-male, as from the beginning of the play Blanche has threatened this position, she has drunk his alcohol, manipulated his wife and this pinnacle marks the end battle of this ongoing power-struggle. Stanley is not belittling Blanche or women in general by raping her rather he feels it is his last resort in order to have victory thus proving he sees her as a genuine intimidation. Williams challenges the traditional view of marriage as posed by cultural standing, assuming that marriage is the end of the story, and that marriage is synonymous with a lifetime of joy, erasing all pain from behind that moment. Williams sees marriage as the start of a life for a man, far from the bliss that is advertised. Mitch and Stanley represent realistic portraits of men who will constantly cram their women into gendered stereotypes and Stella and Blanche will always oblige to protect their own stature and (in the case of Stella) to shelter their children.

In many cases when people talk about carrying things they speak about physically carrying an object with some amount of weight from one place to another. Many times however people carry things with them throughout life that have no physical weight, weighing themselves down with the â€Å"heavy† burdens that life brings.Both Wideman and Obrien’s short stories exemplify a common theme of persevering through struggles and relieving oneself of the weight of life’s struggles. The soldiers in O’Brien’s short story â€Å"The Things They Carried† carry heavy physical loads necessary for them to survive out in war, but they also carry heavy emotional loads which will be with them for the rest of their lives if they are unable to let them go. Some things the men carry are universal, like a compress in case of fatal injuries and a two-pound poncho that can be used as a raincoat, groundsheet, or tent.Most of the men are common, low-ranking soldiers and carry a standard M-16 assault rifle and several magazines of ammunition. Several men carry grenade launchers. All men carry the figurative weight of memory and the literal weight of one another. They carry Vietnam itself, in the heavy weather and the dusty soil. The things they carry are also determined by their rank or specialty. Each mans physical burden consisted of weapons, cigarettes, C rations, and packets of Kool-Aid, and the more intangible things, such as fear and silent awe, that weigh these soldiers down.As leader, for example, Lieutenant Jimmy Cross carries the maps, the compasses, and the responsibility for his men’s lives. The medic, Rat Kiley, carries morphine, malaria tablets, and supplies for serious wounds, and the responsibility to save lives. The things they carry depend on several factors, including th e men’s priorities and their constitutions. Because the machine gunner Henry Dobbins is exceptionally large, for example, he carries extra rations; because he is superstitious, he carries his girlfriend’s pantyhose around his neck.Nervous Ted Lavender carries marijuana and tranquilizers to calm himself down, and the religious Kiowa carries an illustrated New Testament, a gift from his father. With the amount of space that the author gives to enumerating the weight of these objects, one might assume that these objects are what are really important to these soldiers, but in reality it is the incalculable weight of their burdens that truly weigh them down. The â€Å"things† of the title that O’Brien’s characters carry are both literal and figurative.While they all carry heavy physical loads, they also all carry heavy emotional loads, composed of grief, terror, love, and longing. Each man’s physical burden underscores his emotional burden. Henry Dobbins, for example, carries his girlfriend’s pantyhose and, with them, the longing for love and comfort. Similarly, Lieutenant Jimmy Cross, of the Alpha Company, carries various reminders of his love for Martha, a girl from his college in New Jersey. Cross carries her letters in his backpack and her good-luck pebble in his mouth.He carries her photographs, including one of her playing volleyball, but closer to his heart still are his memories. Lavender, one of the soldiers in the story, gets shot on his way back from going to the bathroom. That night the soldiers sit in the darkness discussing the short span between life and death in an attempt to make sense of the situation. The morning after Lavender’s death, in the steady rain, Cross crouches in his foxhole and burns Martha’s letters and two photographs.By burning the physical reminders of Martha Cross believes that he will be able to forget about his past with her, and stop fantasizing about their future. O’Brien wrote â€Å"Besides, the letters were in his head. And even now, without photographs, Lieutenant Cross could see Martha playing volleyball in her white gym shorts and yellow T-shirt. He could see her moving in the rain. † Even without the pictures and the letters he was still carrying Martha. These emotional burdens are the heaviest because they are intangibles and therefore cannot be disposed of.Physical burdens are no more than that; if necessary they can be discarded. Emotional burdens, on the other hand, must be endured. O’Brien, speaking of cowardice in particular, says, â€Å"in many respects this was the heaviest burden of all, for it could never be put down. † The soldiers know there is no easy way to rid themselves of their fears because of their abstract nature, but they dream escapist dreams of flying away in a plane and â€Å"falling higher and higher,† free of weight.Jimmy Cross tries to rid himself of intangible burdens by di sposing of tangible ones that, to him, represent intangible qualities. He does this by burning his letters from Martha. He knows, though, that this simple act cannot rid him of his memories. â€Å"He realized it was only a gesture†¦ Besides, the letters were in his head. † His love for Martha is also represented by the small pebble, which she gave him, but the easily disposable pebble, which weighs merely an ounce, represents a much heavier emotional burden that he cannot rid himself of.Though in Wideman’s short story â€Å"Newborn Thrown in Trash and Dies† a tiny baby is cast down a rubbish chute with no tools to survive, no physical load except for her own weight, she carries a heavy emotional load and reflects on what her life might have been had she lived on each floor of the tenement building where her 19-year-old mother lives. In the first paragraph of the story Wideman quickly expresses the theme of carrying burdens. Wideman writes, â€Å"Your life rolled into a ball so dense, so super heavy it would drag the universe down to hell if this tiny tiny lump of whatever didn’t dissipate as quickly as its formed.Quicker. The weight of it is what you recall some infinitesimal fraction of when you stumble and crawl through your worst days on earth. † Here the newborn speaks about burdens and mishaps that come about in life. She explains to the reader that she will not be able to receive much of a life but that people would have nothing to live for if they did not forget about the struggles and problems that were flashed before their eyes before they were born into this world. The rest of the short story tells a complete play-by-play of the flash of life she had before she was brought into the world.Each floor represents another stage or point in her short life. The floors of this story disguise the days of life, and the newborn that will have no chance to experience them explains the days of life perfectly in these words; â€Å"I believe all floors are not equally interesting. Less reason to notice some then others. Equality would become boring, predictable. Though we may slight some and rattle on about others, that does not change the fact that each floor exists and the life on it is real, whether we pause to notice or not. †People cannot have a good day everyday or everyday would become boring and predictable. In many instances of life people are put into situations such as the war that the soldiers in â€Å"The Things They Carried†, that they have no control over, and that they could not even begin to explain to people for the mere fact that the situation that they are in no one should ever have to think about let alone experience. On the other end of the spectrum good days and good experiences are most often remembered and reminisced about for the rest of peoples lives, which they should be.The thing that people don’t realize is that very often people carry around the burden s of their pasts and the bad days that they have had which make the rest of their lives less enjoyable. After the war, the psychological burdens the men carried during the war will continue to define them. Those who survive will carry guilt, grief, and confusion, although the heavy backpack filled with tools to survive will be gone. In both stories the characters carried emotional burdens, the soldiers carried fear and hope as well as the newborn baby.The soldiers hoped to see another day, and were scared that the opportunity might not come. They had lived lives before the war and feared that they might never get the opportunity to live happily with their loved ones again so they carried belongings of their loved ones physically trying to keep their loved ones close and not forgotten. The same holds true with the newborn girl. She never gets the opportunity to experience her family, or to even establish a connection with anyone before she dies. Still she fantasizes about what it mig ht have been like, what might have happen.The emotional burdens of fear of death seem to be unbearable for the soldiers mainly because they know that they are losing the opportunity of life. The newborn however doesn’t seem bitter about dying, she feels sympathy for the mother who put her in the trash and accepts her life as being â€Å"how it is,† as she doesn’t know any better. All in all the characters of both the stories carry their emotional loads till death, or until they go back home which even then the psychological affects of the war will still haunt them until they learn to let them go.So as the newborn surrenders to her death she lets go of her emotional burdens floor by floor never looking back, so to should the soldiers realize that their days are numbered and tomorrow is never promised so just as Wideman wrote at the end of his second paragraphs about how people try to forget the flash of their life that occurs before they are born people should al so try to forget the bad days and the burdens of life as they happen and, â€Å"live your life as if it hasn’t happened before, as if the tape has not been punched full of holes, the die cast. †
